Are You Ready to Begin the Admissions Process to Providence Ministries Recovery?
Congratulations on your decision of seeking a new way of life free from addiction. We have made the process of gaining admittance to the Providence Ministries men’s addiction recovery program as simple as possible and ask that you read every word below to understand what to do next.
Step One - Interview
The very first thing you need to do is call us at phone number 706-275-0268 and ask to schedule a phone interview for entry into the men’s addiction program. Upon completing this short interview and having been determined to be a good fit for our program, you will proceed to step two.
Step Two - Download, Fill Out, Fax Back Application
The second thing you need to do is download the Admissions Application below. Fill it out as thoroughly as possible, print and sign it, and then fax it back to us at:

Step Three - Get Blood Screen
After we’ve received your completely filled out and signed application, you will be instructed to have blood tests performed. The types of tests that need to be performed will be disclosed to you during or after the phone interview. After we have received your blood test results, you will proceed to step four. Step Four - Schedule Entrance Date
